UrgentCare2Go® offers in-home and virtual Transitional Care Management (TCM) services to help patients safely transition after discharge from hospitals, skilled nursing facilities (SNFs), or other inpatient settings. We partner with hospitals, case managers, primary care physicians, and specialists to reduce readmissions, improve outcomes, and enhance patient satisfaction.
What Is TCM?
Transitional Care Management ensures patients receive timely follow-up after discharge to avoid complications and readmission. We provide:
Interactive contact within 2 business days of discharge
Face-to-face visit (in-home or virtual) within:
7 days for high-complexity (CPT 99496)
14 days for moderate-complexity (CPT 99495)
Medication reconciliation and care plan coordination
24/7 clinical support access after enrollment
